Care Coordinator (Safeguarding, Immunisation & Palliative Care)

Aspen Medical Practice is seeking a motivated and organised Care Coordinator to support our safeguarding, childhood immunisation and palliative care services.
This newly created role offers a genuine opportunity to make a difference to patient care while working closely with GPs, nurses, community services, partner organisations and other healthcare professionals.
We are looking for someone who is compassionate, well organised and able to build positive working relationships. Salaried General Practitioner (GP)
4-8 Sessions per Week | Permanent | Flexible Working Supported | Health and Care Worker Visa Sponsorship Available
10 appointments. 15-minute consultations. 1 hour 10 minutes protected administration time.

Aspen Medical Practice is seeking a Salaried GP to join our friendly, supportive and forward-thinking team. We believe excellent patient care depends on giving clinicians the time and support they need to work safely and sustainably.
Our standard GP clinics include ten 15-minute face-to-face appointments, four protected follow-up telephone slots and 1 hour 10 minutes of protected administration time per session. We care for approximately 32,000 patients and have invested in a large multidisciplinary team, dedicated administrative support and structured appointment templates to help reduce GP workload and support continuity of care. GPs work within a collaborative Duty Hub model, providing access to colleagues for advice and support throughout the day.
We welcome applications from both UK-based and international General Practitioners. Aspen Medical Practice is a licensed UK visa sponsor and can provide Health and Care Worker visa sponsorship for eligible candidates.
This is a permanent role for 4-8 sessions per week. Flexible working arrangements will be considered, and we are happy to discuss individual working patterns with suitable candidates. We are particularly interested in candidates who can work on Mondays and/or Wednesdays. Applications from doctors able to contribute to our Extended Access service through an evening or Saturday session are particularly welcomed, although this is not an essential requirement.

Paramedic Practitioner / Nurse Practitioner

Two permanent, full-time (1.0 WTE) roles
Working pattern: 8.00am–6.00pm, with one 10.00am–8.00pm enhanced access shift each week
Salary: Dependent on experience
These are varied and rewarding roles for clinicians who are passionate about providing safe, compassionate and patient-centred care within general practice.
You will work alongside our established team of Paramedic Practitioners, GPs and wider multidisciplinary team. Your role will include assessing and managing patients presenting with a broad range of minor illnesses, as well as undertaking home visits as the practice’s visiting clinician, with GP support available when required.
We are keen to support the successful candidates in continuing to develop their clinical knowledge, confidence and scope of practice.
Who we are looking for
We are looking for two experienced and motivated clinicians. Successful applicants may be either Paramedic Practitioners or Nurse Practitioners and must have previous experience of working in primary care.
Applicants must have completed a minor illness course and a prescribing course. They must be confident working autonomously within their professional competence while recognising when to seek advice or clinical support.
The successful candidates must be prepared to work in minor illness clinics and undertake home visits as the practice’s visiting clinician, with GP support available. They must also be able to work one enhanced access session each week, although the day can be agreed flexibly.
We are particularly keen to hear from clinicians who value teamwork, are committed to safe and compassionate patient care, and are willing to continue expanding their clinical skills and the range of patients and conditions they are able to assess and manage.
